SPRINGDALE — Jo Ann Putman Mayfield Boggs, 65, of Springdale died Dec. 27, 2004, at Northwest Medical Center of Washington County in Springdale. She was born May 15, 1939, at Pea Ridge to Rex Mabry and Ada Marie Haynes Putman. She was a lifelong resident of the
area. She attended Old Missouri Road Baptist Church in Springdale and was a member of Beta Alpha Sorority and the DAV Auxiliary.
Survivors include her husband, Charles Boggs, of the home; a son, Greg Mayfield and wife Sarah of Springdale; one daughter, Diana Mayfield Thomasson and husband Kevin of Pea Ridge; two stepsons, Charles William Boggs Jr., of Orlando, Fla., Richard Boggs and wife Paula of Woodbine, Kan.; a stepmother, Bea Putman of Texas City, Texas; two sisters; two stepsisters; one stepbrother; nine grandchildren; 11 great-grandchildren; one great-great-grandchild.
Services will be at 1 p.m. Monday at Sisco Funeral Home Chapel in Pea Ridge with Ron Morris officiating. Burial will be at Pea Ridge Cemetery. Arrangements are by Sisco Funeral Home of Pea Ridge.
Visitation will be from 2 p.m. to 4 p.m. Sunday at the funeral home.
