SILOAM SPRINGS — Tammy Kaye Perkins, 35, of Siloam Springs died Dec. 3, 2010, at Siloam Springs Memorial Hospital. She was born May 19, 1975, in Wauchula, Fla., to Edward Lee and Sandra Kay King Perkins.
She is survived by her son, Taylor Lee Perkins of Siloam Springs; her father, Edward Lee Perkins of Siloam Springs; her mother, Sandra Kay Walker and husband Wayne of Elm Springs; a brother, Anthony Lee Perkins of Gentry; and a sister.
Visitation will be from 6 to 8 p.m. Monday at Heritage Funeral Home in Springdale.
A graveside service will be 1 p.m. Tuesday at Elm Springs Cemetery.
