Judith Kay “Judy” Pervis, 65, of Siloam Springs, Arkansas, died August 2, 2012, at Willard Walker Hospice, Fayetteville, Arkansas. Born December 30, 1946, in Fort Worth, Texas, she was the daughter of Orvil Haddock and Lee Ellen Alexander Haddock. She was a homemaker and moved to this area in 1984. She attended the Seventh-day Adventist Church in Gentry. On December 23, 1965, she married Norman L. Pervis.
She was preceded in death by her parents. Survivors include her husband Norman of the home; two sons, Norman Lynn Pervis and wife Nancy of Grand Prairie, Texas, and Sam Pervis and wife Stephanie of Siloam Springs, Arkansas; one daughter, Kim Spells and husband David of Sachse, Texas; grandchildren, Cameron, David, Gigi, Landon, Camden, Anthony, Tianna, and Alex; one sister, Karen Putnam of Keene, Texas; and one brother, Bob Haddock of New Jersey.
