Ofelia Reyes, 58 year old resident of Siloam Springs, Arkansas, died December 21, 2017, in Tontitown, Arkansas. Born January 19, 1959, in San Jose Guayabal, Cuscatlan, El Savador, she was the daughter of Leoncio Acosta and Martina Filomena Navas. She married Jose Luis Reyes. Ofelia worked for Simmons Foods Inc.

She was preceded in death by her father, her husband and one sister and two brothers.

Survivors include her sons Luis Reyes and wife Nicole of Old Hickory, Tennessee and Jorge Reyes and wife LaTara of Gentry, Arkansas; four grandchildren; her mother Martina Filomena Navas; sisters Enma Acosta DeMontes, Maria Elba Acosta Mejia, Lidia Acosta Rodriguez and Berta Alicia Acosta; brothers Miguel Angel Acosta and Jose Adan Acosta.

Funeral services will be 10:00 a.m., Wednesday, January 3, 2018, Gentry Seventh Day Adventist Church, Gentry, Arkansas. Burial will follow at Oak Hill Cemetery, Siloam Springs, Arkansas. Visitation will be 6:00 p.m. to 8:00 p.m., Tuesday, January 2, 2018, at Wasson Funeral Home, Siloam Springs, Arkansas.

TONTITOWN — Police have released the names of the two people who died in a two-car collision at 10:24 a.m. Thursday.

Marcelina Cox, 72, and Ofelia Reyes, 58, both of Siloam Springs, were in one of the cars and pronounced dead at the scene, according to a preliminary vehicle crash report by the Tontitown Police Department.

Trisha Ann Caldwell, 28, of Siloam Springs was in the other car, according to the report. Central Emergency Medical Service took Caldwell in serious condition to Washington Regional Medical Center in Fayetteville. The hospital didn't respond to a request Friday morning for an updated condition.

The wreck happened on Henry De Tonti Boulevard.

The vehicle with Cox and Reyes was traveling west on the highway in the slow lane and then took a hard left into the eastbound lane, according to the report. The car passed in front of Caldwell's car and Caldwell's car hit the passenger's side. It was a T-bone accident.

There were no witnesses to explain the reason for the hard left turn, according to the report.
