Jack Eric Fay, born on November 11, 1984 in Houston, Texas, passed away on January 7, 2010, at his home in Appleton.
He is survived by his father, Myron D. Fay, his mother Ladona Chacon, one brother, Benjamin Fay and other relatives and friends.
Services were held at the Appleton Presbyterian Church with Rev. Randell Williams officiating. Burial was in Appleton Cemetery at Appleton.
